can someone explain the difference for me - my provider says they are one in the same??

The basic difference is the region.
846.0[Lumbosacral (joint) (ligament) sprain]
The code 846.0 is under heading of code 846.xx which is for term "Sprains and strains of sacroiliac region"
Sacroiliac is region of the joint where the sacrum and ilium meet.

847.2 [Lumbar sprain]
The code 847.2 is under the heading code 847.xx which is for term "Sprains and strains of other and unspecified parts of back"

Firstly, Lumbosacral sprain is not exactly same as Lumbar sprain.
846.0 indicates the Lumbosacral region that is mentioned with additional parenthetical information about region saying:joint and ligament.
847.2 refers to the Lumbar region in the context of 847.xx category that refers to "other unspecified parts of the back" in the Lumbar region that is not covered in the region defined by the code 846.0
