We are performing epidural steroid injections (transforaminal and caudal) in our office with fluoroscopic guidance. Would it be appropriate to bill 77003 or 72020-TC? We save the images in the chart and the use of fluoroscopy is documented in the procedure report. Does a radiologist have to read for either of these codes? Thank you!

The regular epidural steroid injection (ESI) procedures represented by codes 62320-62323. These ESI codes specified whether the provider did or did not use imaging guidance. So, it is not appropriate to code fluoroscopic guidance separately. Radiologist does not have to read for either of these codes. Physician performed fluoroscopy, a live X–ray where the image appears on a monitor, to view body structures while performing procedures.
