Can 69210 be billed with an e/m code 99213 when the sole reason for the vist was ear pain/ hearing loss or is the 99213 bundled into the 69210

Yes, since the reason of visit(ear pain/hearing loss) is different from 69210(impacted cerumen), you can bill 99213-25 along with 69210
