After surgery can the doctor do a revision and charge for it, or is the repair included in the original fee?

Repair is included in the primary procedure code and it cannot be report separately.

Also, you can bill for revision or repeat procedure if and only if your documentation indicate the medical neccessity for the revision.

Append modifier -76 (Repeat Procedure) OR modifier -78 (Unplanned Return to the Operating/Procedure Room, following Initial Procedure for a Related Procedure During the Postoperative Period) on your CPT Procedural code to bill for Revised procedure. 

Note: Please make sure, your documentation clearly mentioned the reason why the procedure is been revised/repeated during Post-operative period.
