If a pt has a history of personal colon polyps V12.72 and are coming back after for a 5 year recall w/no present symptoms, do you code the colonoscopy w/preventative V76.51 as the primary dx & V12.72 as the 2ndary or vice versa?

Medicare will allow only select diagnoses to support a high risk classification. One of such criteria is: V12.72 — Personal history of certain other diseases; diseases of digestive system; colonic polyps

This guideline doesn't mention a different criteria for if polyp is resolved or not. So, the appropriate Primary Dx would be V12.72
