Patient presents to hospital with hematochezia, status post colonoscopy w/polypectomy. Dr performs colonoscopy & identifies polypectomy site & attempts to place a Hemoclip, which he is unable to place due to anatomic location. Attempt to place Hemoclip aborted. Would I use 45382-53 for the attempt or 45378?

Well, CPT 45382-53 is appropriate code to bill for the scenario. Modifier 78 can be use for unplanned return to the operating room. Hope it helps!
