31652/31653 EBUS Coding questions
With the old code we would bill per anatomic lung Lymp node locations (so if bx in 4R and 10R) you would only bill 1 biopsy.
With the new codes do we still go by these locations or is it per Biopsy , regardless of the anatomic location?
Example: would this be coded as 31652 or 31653?
Specimens Removed: Transbronchial Needle Aspirate with EBUS guidance - 7, 4R and 10R
31652 – one or two mediastinal and/or hilar lymph node stations or structures
31653- 3 or more mediastinal and /or hilar lymph node stations or structures

Greetings from SuperCoder.com!
Thanks for your question. Here is the resolution to your query:
The 2017 code set revises codes 31652 and 31653 by removing moderate sedation, also called conscious sedation, from this procedure. Use of moderate (conscious) sedation is no longer considered an inherent part of this procedure and you can now report it separately. You should report the codes separately, according to the number of lymph node stations explored.
Use code 31652 when the provider uses endobronchial ultrasound–guided bronchoscopy to obtain tissue and fluid samples of 1 or 2 mediastinal and/or hilar lymph node stations or structures for laboratory analysis.
Use code 31653 when the provider performs bronchoscopy with endobronchial ultrasound to obtain tissue from 3 or more mediastinal or hilar lymph node stations and structures.
Please feel free to write if you have any concern or questions.
