27680 vs 27681

Provider did tenosynovectomy on 3 different tendons (digitorum longus, posterior tibial and peroneus longus tendon. He made 1 incision for the first 2 tendons and a different incision for the last one. What CPT codes would you use??

Hi, According to the scenario, we would code CPT 27681 as the provider treats multiple tendons through two separate incisions for the procedure. When tenolysis of multiple tendons is performed through a single incision, we report the code 27680 only once as single tenolysis. However, in this scenario the provider makes two incisions for three different tendons, so we would report only 27681. Hope this helps! Thanks.
