For 2012 there is the new code 22633 for the combo of 22612 and 22630 at the same levels. My question is when there are additional levels of one or the other fusion code. Example: if you did a interbody fusion at L4-5 and the posterolateral at L1-2, L2-3, L3-4, and L4-5. Would you use the 22633 for L4-5 and then 22614X3 for L1-2, L2-3, and L3-4 or would you use 22633 for the L4-5 and then 22612 for L1-2 and 22614X2 for L2-3 and L3-4? And then what if same scenerio but you also had the addition of the interbody fusion at L5-S1, would this be the 22630 or 22632?
