2019 CMS CHANGES TO E&M

Do you have suggestions as to how a provider should indicate he has reviewed and verifies the HPI? Of course, a templated sentence such as, "I've reviewed and verify the documented HPI." would work. Would this need to be below the HPI or would at the end of the document suffice? It seems one type of work is being switched for another for the provider. The verbiage "I've reviewed and verify the documented HPI" you mentioned should be used in proivder note that can be below HPI or at the end of document.
