19370, 19371, 19380 and 11970 on same breast same session

patient had capsulotomy (top of breast) 19370, capsulectomy (bottom of the breast) 19371, revision of reconstructed same breast (side of the breast) 19380 and exchange of TE to gel implant (11970). Most of these codes are usually bundled. If so, which codes would be billable or what’s the best way to bill this procedure.

Amongst the codes mentioned by you, only codes 19371 and 19370 are bundled as per the CCI edits. Append modifier 59 to unbundle these codes.
