Our Radiologist wants to put a clip on the area that is being operated. Patient is scheduled for an upcoming surgery with a wire localization outside of our facility. They too will be using this code to put the wire in. Will Medicare cover this code if both facility are using it?

Thanks for your query.

Placement of breast localization device (clip/wire/needle etc) are site dependent codes. If additional clip or wire is placed in a separate site/lesion, additional placement to be coded with appropriate add-on code.
If wire and clip placement are done on the same lesion/site, it would not be appropriate to code placement twice. If clip and wire are placed in separate lesions, procedure can be billed separately once with primary code and secondary placement with add-on code.
If image guidance is used during wire/clip placement, please read section guideline on pg. 90 of AMA CPT 2014 book to code correctly.
