14060 Adjacent Tissue Transfer/Rearrangement

This was two distinct lesions, one on each side of the nose that required two complex flaps. The area was less than 30sq cm in total however. How can I code this for what I consider 2 separate procedures.

Can the above service be reported with 14060 and 14060-59 for the 2nd?

I am Confused . What is the size of first lesion? and second lesion also. Is the total area less than 30 sq cm without mentioning individual sizes. Please explain.

He does not specify individual sizes for each lesion only states that combined they do not equal 30 sq cm. He had originally wanted to report them with modifier 50 and I told him this would not be appropriate. That is when I received the message above.

So we have to code with assumptions taking minimum considerations.

We have two lesions

First 10 sq cm or less--14060
Second 10 sq cm or more-14060 ( for first 10 sq cm) and 14061 for the remaining. ( Its less than 30 sq cm)
