CPT® Codes — Category II Codes

This code range includes CPT® Category II codes. The American Medical Association (AMA) maintains the Current Procedural Terminology (CPT®) code set.

CPT® Category II codes are supplemental tracking codes used for data collection related to performance measures. These five-character codes end with F, such as 0001F. Reporting these codes may be optional.

Divisions include composite measures (e.g., assessment of areas covered by multiple other codes), patient management (e.g., prenatal care or dialysis), patient history (e.g., tobacco use in patients with specific diagnoses), physical examination (e.g., assessment of asthma impairment), diagnostic and screening processes (e.g., documenting screening mammography results), interventions (e.g., prescription of beta blockers in a patient with heart failure), follow-up and outcomes (e.g., providing an asthma discharge plan), patient safety (e.g., querying a patient about falls), structural measures (e.g., entering patient information in a reminder system), and non-measure claims based reporting (e.g., aortic aneurysm seen on CT).

Note that AMA resources label these Category II codes rather than Category 2 CPT® codes.
