Avoid Going Crazy Over KZ

Question: Have you ever heard of modifier KZ? We have a claim being denied because we "failed to use the KZ modifier." I am at a loss.

Answer: Modifier KZ means "New coverage not implemented by managed care." You need to use this modifier on implantable cardioverter defibrillators (ICDs) placed on Medicare Managed Care claims for the new coverage guidelines. However, make sure you report implants performed under the older guidelines without this modifier. You should apply modifier KZ to the appropriate ICD code.
