If the CPT code in a reimbursement filing is not currently cross-referenced with a targeted ICD-9 code, will the reimbursement filing be automatically denied? If so, how does one get the proper cross-reference established?

The CPT should be mapped to correct ICD-9-cm code as per the list of diagnosis codes in the LCD of the said local medicare contractor.
