This denial code: M80 was given by medicare for CPT codes: '62311' and '20551 - 59 - LT'.
*No E/M was billed on that day.
The Diagnosis codes were checked and meet the LCD criteria so it's not the ICD codes. Neither CPT was paid.

I think this is a case of billing multiple E/M service on same date of service. This will only be covered if performed in different date of service. No modifier can help reimbursement if billed on same date of service.
