   where do you locate the "types of service" for instance surgery is type 2, office exams are type 1? I need to code a sperm check, 89321 but am unsure what the type of service would be

   Hi - Check out pp30-31 of the claims processing manual. http://www.cms.gov/manuals/downloads/clm104c26.pdf. E.g., it has a 5 for Diagnostic Lab.
