  1. We have therapists in our hospital that provide selective and non selective wound care (97597/97602). Our therapist use Silvermed Gel A6248 when doing the wound cares on some of the wounds. We are wondering if this is appropriate to bill since the codes 97597/97602 states "including topical application". There is no reference as to what that may include. I have check several LCDs and still not sure on if that is a billable item. We also do not have a supplier number for DME. Thank You!

  2. Topical application is included in both, so is not a factor to differentiate if it is a selective wound care or non-selective wound care procedure.

    The basis of selective and non-selective wound debridement procedure depends on the method of care taken.
    If the debridement procedure, including topical application, also include application of high pressre waterjet, use of a whirlpool and code is based on area of debridement, it is selective debridement.
    *
    If the debridement, including topical application doesn't involve the significant features of selective debridement(high pressure debridement technique), but involves wet-to-moist dressings, enzymatic abrasion, etc including topical application and the code is not based on area but by method as described, then it is non-selective debridement.

  3. Infected areas particularly cellulitis, pressure ulcers, and ulcers are compatible for these CPT codes.
