    A hospital here in our town is now requiring our GI physicians to perform a full H&P for all patients entering the hospital for outpatient Remicade infusions. Will this be a chargeable service for our physician or will it be considered global to the infusion service since the infusion is the service intended for the patient's outpatient hospital visit?

    If this is only H/P , then it will be inclusive.

    If your physician performed separately identifiable E/M services (have medical necessity) then it would be billed for physician.

    Is this also correct if our physician does not perform the injection, he is only ordering it? The injection is done by the outpatient facility at the hospital.

    I assumed it would be, but wanted to be sure...

  2. Yes you are right. Even if your physician is ordering the injection but given in the outpatient facility by someone else, still it is to be considered incidental to the infusion services as the H&P service provided here is part of a policy to be performed before Remicade Infusion. Had there been an E/M service significantly separate distinct service not related to Infusion service, then we could have coded the E/M service.
