  1. How would I code 3 pterygium excisions on the LT eye - 2 medial and 1 lateral? Also, my physician is stating that he has been coding pterygium-eye modifier but not indicating number. Is this OK?

  2. The excision of pterygium with graft (65426) has a multiple procedure indicator of “2”. If performed on the same day as an excision of eyelid lesion, multiple surgery rules apply. The majority of ophthalmic surgical procedures have a multiple indicator of “2”.
    2 = Multiple procedure rule applies.
    So, Modifier 51 should be used accurately.
