  1. I use code 27860 for manipulation when doctor does an injection and manipulation under general anesthesia of ankle joints. That code does state ankle therefore is it appropriate to use for manipulation of the MTP joint? I do not see a code for manipulation of the digits.

The note states, joint was manipulated into inversion, eversion, dorsoflexion, plantar flexion and vargus/valgus.

  2. CPT 27860 is for manipulation of ankle only, not MTP joint. Metatarsophalangeal joint is different from ankle joint even though both are in lower limb. There is no separate code for exact manipulation of MTP joint because in Leg section, there is a single code for manipulation 27860. If doctor is doing manipulation for MTP joint, 27860 is inappropriate, I think. If doctor is doing closed treatment of fracture and/or dislocation with manipulation of metatarsal bone, you could use 28475 for each. If it is not appropriate, you should go for 27899.
