  1. Ok, so... I was instructed when there is suspected disease in the subclavian artery we can code for the selective placement of the catheter, (32615-32617) but when the arteriogram is done to check to see if it a viable conduit for bypass then it is considered included in the cath code.
    With the new codes for 2011, 93459, is that also the case? I have received denials and appealed them to mixed responses.
    I need something to be able to reference to back up the reasoning.
    Any help is greatly appreciated...

  3. If I read this correctly, all catheter placements are included when a heart catheterization is performed on the same DOS?
