  1. Microdiscectomy via transforaminal approach at L4-5. Soft tissue were bluntly dissected down the facet joints at L4-L5 under direct visualization,utilizing multiple fluorsocpic views, spinal needles were inserted into the disc space at L4-L5. Guidewires were placed through the spinal needles and the spinal needles were taken out. An incision was made around the guidewires. The dilators were placed over the guidewires into the disc space. Working tubes were placed over the dilators into the disc space. The guidewires and dilators were taken out. Using the treansforaminal approach, the endoscope was inserted, using pituitaries and graspers, microdiscectomy was performed.
