   How do we code two separate excisions '19301' of two cancerous lesions in the left breast? There were two incisions on the same breast. Would you use 19301, and 19301 with the modifier 59? How do you show it was the same breast - they are going to think there was a lesion in each breast...

   You can use modifier LT with each, but by using 59 additionally it will justify that this is another lesion on the same breast.
