   how would you code full-thickness sharp debridement, right heel down to the level of bone, 6 x 6 cm in size? the op report says "there was a gangrenous heel ulcer. With scalpel blade, the necrotic tissue was excised. There was exposed bone." Would you code that as a '00400' or '01470'?

   It would be 01470, not 00400, since the debridement/excision went till bone level. By the way, both 00400 and 01470 has 3 BU, so chance of losing $$ even if you choose the other one. 00400 would have been appropriate if it was done till a shallower level.
