   Our physician dictated he did a right shoulder deep heterotopic ossification excision. He used a osteotome to free it up from the coracoid as well as the clavicle and the AC joint and then was excised with a rongeur. What CPT would you recommend?

   1st alternative: You need to go by complete documentation, if it part of any comprehensive procedure, then you can use modifier 52 with that CPT. If it is closest to any cpt, but less than this documentation, then you can use modifier 22 with that CPT.
