   Need to get the correct cpt & modifier. My surgeon billed out on 10-5-11 a 49507- ing hernia incar,49560 ventral her, 49568-mesh,but on 10-16-11 the pt came back to the hosp w/infected ventral mesh he wants to billed out 49565-78,22 & 49521-78,22 with cpt 11008 can I bill out the 11008 or is it included with the recurrent hernias? He did say he took out the infected mesh and repaired the hernia again.

   CPT 11008 can only be billed in addition to 10180, or 11004, or 11005, or 11006. We can't bill 11008 without billing the above primary CPTs.

   We should code 49999 unlisted code and bill with an equivalent cpt code 11008.
