  1. My doctors do the above procedure in our office and we own the eqiupment. We bill out '76831', '58340' and '76830'-ultra sound transvaginal. My doctor feels that we should be able to bill the '76830' twice because the probe is incerted twice during the procedure. Would this be correct?

  2. There is high probability of denials in such cases.
    Reason: The MUE for 76830 is "1". It is accepted that bypassing MUE audits can be possible with modifiers like 76, 59, etc. But, there must be a different reason of medical necessity when it is performed twice on the same session, and similarly there must be medical necessity to perform twice in a day with two different sessions, but here it is only one sesion.
