  1. I have coded 94060, 94240, 94260, 94360, and 94720. 94260 is being denied. The insurance company is stating that this is bundled with CPT code 94240. There are no CCI edits for these codes. Does anyone know where I can get documentation regarding this issue?

  2. Can you tell me the Insurance which denied ?

    I have got info from the website below which a specific denies 94260 with 94240 for being mutually exclusive. May be this denial is insurance specific, as most of the supporting Coding websites don't show edits for these pair of codes.

  3. This insurance company is Assurant, but I have seen others. I am just not able to find any documentation justifying WHY these codes would be billed together.

  4. the only codes u can use for PFT billing are: 94060, 94240 and 94720according to most insurances. and it depends

    on your pft machine , if it has a chamber or not and if u measure the resistance of air flow(94360)
