Procedure was Ilionguinal nerve post herniorrhaphy neuroma radiofrequency and phenol neurolysis. I figure code 64640 but could not find a code for the phenol injection. Below is part of the note:
Using continuous fluoroscopic imaging 1 mL of myelogram safe contrast dye was injected showing localized spread in the area of the previously identified neuroma. This was then followed with 1.5 mL of 6% phenol under continuous fluoroscopic guidance to be sure that there was no vascular uptake and that the phenol remained localized to the area of the identified neuroma.
