  1. I need to find in writing the rules on billing for a medical assistant, and/or a registered nurse when the physician is NOT in the office.

  2. You are not allowed to bill for med assitants when the physician is not in the office. There is no incident to that applies

  3. for example 29580 - if physician gives order for medical assistant/nurse to apply unna boot does he need to document or can the medical assistant or register nurse document that the unna boot was applied per Dr XYZ
    and then Dr XYZ sign note? as long as he is present within office suite
