  2. Do you mean 76775? If so, 76775 has a bilateral status indicator of 0 in the Medicare Physiian Fee Schedule whch means the 150% bilateral payment adjustment doesn't apply. Payment for two sides (50 or RT/LT) is lower of (a) actual charge for both sides or (b) fee schedule amount for single code. Therefore, you should report 76775 without modifier 50.
