   My Doctor was the assistant surgeon but also did the nerve block 64450
   Do I need a modifier? Most insurances have paid without a modifier but some have denied it as being inclusive.

   Generally we don't need to add modifier for the above case.

   But it depend upon the another procedure done with 64450 on same DOS.
   If some other major procedure was done along with 64450, then check the CCI edit for the applicability of 64450.
