CPT 2013 deleted codes 95904 and 95903. I believe the table you are looking at is the deleted code edits table. CCI 19.0 shows:

Codes 95904 and 95903 were deleted to introduce new codes that specify the number of studies represented by the code, from one to 13 or more. Familiar test codes 95900-95904 were deleted from CPT® 2013 in favor of seven new options including:
95907: Nerve conduction studies; 1-2 studies
95908: Nerve conduction studies; 3-4 studies
95909: Nerve conduction studies; 5-6 studies
95910: Nerve conduction studies; 7-8 studies
95911: Nerve conduction studies; 9-10 studies
95912: Nerve conduction studies; 11-12 studies
95913: Nerve conduction studies; 13 or more studies.
If you use SuperCoder's CCI Edits Checker and select the current CCI version 19.0, effective Jan 1, 2013, you will receive the message that codes 95904 and 95903 are invalid. If you select CCI version 18.3, effective Oct. 1-Dec 31, 2012, you will see the same bundle edit you refer to above with codes 95904 and 95903 showing as active and bundled into 95864.
