A Morgagni hernia is one of the congenital diaphragmatic hernia. It is rare than the Bochdalek hernia and accounts for approximately 2% of all congenital diaphragmatic hernia. Yes. There is no specific CPT code for Morgani hernia with MESH. You can use unlisted 49659 for this. All other codes are fine here but for Nissen fundoplication, code would be 43280/43283 based upon documentation. If documentation supports only for fundoplasy using a laparoscope (Nissen), then you could code only 43280, but it can’t be reported with 43282 because 43280 is an integral part of 43282. If additionally esophageal lengthening is performed in order to extend a short esophagus prior to a separately reportable fundoplasty, then you should go for add-on code 43283.
