  1. 366.03 - I'm trying to use this diag. code with procedure codes 92004 & 92015 and I'm getting error (Other Accident Illiness date is missing.

  3. Please check in the previous claims. Perhaps the patient has been recently or still undergoing treatment for any accident, and the Insurance in the claim form is filed with "Workers' comp" perhaps, so we need to change the Insurance which should not be "Workers' comp" for the present claim.
    Either the above reason or such similar reasons have raised the above errors. Please check the Insurance to which this claim was billed.

  4. This is a new diagnosis that we haven't ever used before. The claim is for an exam and the insurance is
    Medicare. The diagnosis is what is causing the problem. It's kicking out the error when I run it through the
    edit check. So the claim isn't even getting to the insurance company. We just haven't ever used diagnosis code 366.03.

  5. The error you raised last time compelled me to analyze the rejection on that angle. You can also try out by changing the diagnosis code as 366.03 is truly not compatible.
