  1. The surgeon here did two finger fusions and a puckered scar. I included the description of the procedures: left 3rd and 5th DIP fusion and Herbert screw fixation with local bone graft and scar release at the distal portion of the old CMC incision with the release of the extensor pollicis brevis proximal stump from the scar tissue. I thought for the fusions it would be 26860, 26861, 26862, 26863 with icd 9 715.14 would i code all of those since there is 2 fusions? what would i code for the release of scar tissue?

  3. AN LONGATUDINAL INCISION WAS MADE JUST OVER THIS DISTAL PORTION OF THE WOUND AND CARRIED SHARPLY THROUGH THE SKIN. THE LITTLE PUCKERED AREA WAS ELEVATED AND BY ELEVATING THE SKIN FLAP AND THERE WAS QUITE A BIT OF SCAR AND A LITTLE BIT OF EDEMA HERE. THE PROXIMAL STUMP OF THE EPB TENDON WAS FOUND TO HAV ADHERED TO SOME OF THE TISSUES IN THE CAPSULAR REPAIR. sO THIS WAS FREED UP AND ALLOWED TO MOVE DISTALLY. fOLLOWING THIS , THE WOUND WA SIRRIGATED. blEEDING HAD BEEN CONTROLLED WITH THE BIPOLAR. THE SKIN WAS THEN REPAIRED WITH SOME 5-0 NYLON INTERRUPTED SUTURES. THAT TENDON WAS LEFT FREE SO SHE WOULD HAVE BETTER MOTION OF THE MP. THIS WOUND WAS INFILTRATED WITH 0.25 PERCENT MARCAINE WITHOUT EPI.
