   11721 denied for 50 : These Are Non-covered Services Because This Is Not Deemed A 'medical Necessity' By The Payer.
   Is there a limit to how often this CPT can be billed?
   is there a modifier that can help?

   Firstly, modifier 50 not allowed for this CPT. Secondly, the denial reason says, "Not Deemed medical necessity". So, you need to check the ICD that you have used for this CPT.
