  1. my physician has a vacuum assist device that is meant to be use mainly for breast using 19103 but there are times that he could use this device on other things like a node biopsy. In the CPT book the code 19103 states automated vacuum assisted or rotating bx device using ultra sound. If he did a sentinel node bx using cpt 38525 in the office is there a modifier that would need to be attached or is there another code within the CPT book that would pertain to vacuum assist other than the 19103. He said there are many more uses for the vacuum device, however there is not any cpt codes listed in the book for them other than breast.

  2. 38525 is for an open procedure. No modifier describes performing an open procedure as a closed one. I recommend using 38999. If this is new technology, new codes might need to be created. Using unlisted procedure codes will allow data to be captured to hav new ones created.
