  1. I have a question when 64622 and 64623 are performed bilateral should a 50 modifer be added to both procedures? L3 AND L4 OR SHOULD WE ADD LT AND RT?

  2. Yes, for the codes 64622 & 64623 you can apply bilateral rule. But whether you can add modifier 50 to both the codes or LT & RT depends upon payers as that rule is payer-specific. Usually, in case of Medicare, you can append modifier 50 to both the codes. And for private payers, you can write the same code in two different lines with LT and RT. Please contact your payer to make sure which way it should be billed.
