Do you mean to say about the health and behavior assessment or intervention codes. If so then you need to report (96150-96155). If preventive medicine services are performed, then you can additionally code (99381-99397). But you cannot report 90801-90899 and E/M codes 99201-99215 on same day with the health and behavior assessment or intervention codes. For rates check the fee schedule in Supercoder.com
