  1. what is included in E & M Codes - when I code an injection code '96372' with and E & M Code ('99212, 99213, 99214, etc.') it gets kicked back from the insurance stating that '96372' is bundled with E & M Codes and can be used for tracking puposes but must be charged $0.00. This occurs even if you have more than 1 injection at a visit.

  2. In-order to bill an E/M you need to see if the E/M Service is for some different medical condition other than Injection? If that is the condition then you can bill an E/M service by appending a modifier 25. otherwise, if the purpose of visit is only administration of injection then we cannot bill a separate E/M code. Also, if you are using more than one injection at a visit then use modifier 59 with the second and rest other admin code.
