  1. elderly man admitted for coronary ashd. prior cardiac catherization shows 70-100% blocked. Taken to or. coronary artery bypass was performed using 5 venous grafts and 4 coronary arterial grafts.. diagnosis code?
    procedure code?
    any suggestions?

  2. Do codes '33536' and '33522' match your documentation? CPT guidelines offer info on proper reporting of these codes, including which harvesting services should be reported separately. How about 414.0x for ICD-9? Note the "use additional code" instruction for the range related to chronic total occlusion. And watch the graft and native artery distinctions for the fifth digit.
