Do codes '33536' and '33522' match your documentation? CPT guidelines offer info on proper reporting of these codes, including which harvesting services should be reported separately. How about 414.0x for ICD-9? Note the "use additional code" instruction for the range related to chronic total occlusion. And watch the graft and native artery distinctions for the fifth digit.
