  1. Pt here in am and then returned in pm for different dx.

  2. As the patient is visiting your office twice, on a single day, once in AM for a Dx and again in PM for a different and unrelated Dx, you will be able to code both the E&M services separately. Medicare or any other payer will reimburse you in this case. However make sure that you submit proper documentation to support your claim for two separate E&M on same day. Now, if the doctor has not seen this patient in last 3 years, the first visit in the AM will be a code from range 99201 - 99205 (new patient); and the PM visit will be coded as an established patient (99212 - 99215). However, if the patient was already treated in past 3 years, he / she is already an established patient and therefore both the AM & PM visits are required to be coded with "established patient codes" (99212 - 99215).

  3. Sometimes the insurance won't pay for two E/M's performed in a single day, so in this case instead of two E/M codes bill one E/M codes having more code value. Like for Example if 99212 and 99213 then you can code 99214.
