				<description>&#60;p&#62;I do billing for a provider that see's patient's in facility's  he has been doing the 6 min walk stress test on some of the patients and im billing the &#60;code&#62;94620&#60;/code&#62; which medicare keeps dening. ok so I get it now you can't use that code for if patient is in a facility so does anyone have any input on what code I should be billing I am at a loss.&#60;br /&#62;

				<description>&#60;p&#62;We have an established clinic that is going to be opening up it's offices for a separate CHF clinic.  The billing will be separate.  I do the coding/billing for our interventional practice, and my doctor has asked me to find out if other doctors can bill for seeing the patients on the same date of service as the Nurse Practioner, or is the Nurse Practioner the only one who can bill for her services.  All patients will be seen by the NP one at a time and then, as a group, taken to a conference room to hear various drs counsel them about their condition.  How do we bill for this?  Thank you! 				<description>&#60;p&#62;I bill for a neurologist that is going to start interpreting purchased diagnostic radiology studies. I need to know if there is a modifier I need to use to show that the studies are purchased. He will be billing globally and paying the testing facilities for the studies.&#60;br /&#62;
I am also having to clarify for another biller, but if there is documentation somewhere I would love to have. When billing the NCVs, she is just adding a 59 modifier to the 95900 when billing the 95903 for the same nerves. For example, if the tech tested 4 95900 and 4 95903, she is submitting 4 95900 with a 59 modifier and 4 95903. Since I have been billing on the 95903, the new doctor that I just started billing for is now upset because he is being paid less than the other girl who bills, but she's billing incorrectly. I printed off the CCI, but all it says is a modifier would be needed if billed together. Is there any detailed documentation that I can show this doctor and biller that they are not two differnt procedures?&#60;/p&#62;

&#60;p&#62;By chance are there any other billers/coders here who run into Botox injections?  We have patients who are seen for Achalasia/Cardiospasms, Gastroparesis, Eso-Strictures and such that the doctor feels Botox will be a great help for them.&#60;/p&#62;
&#60;p&#62;What is your experience with Medicare on that?  Or any insurance for that type of treatment?&#60;/p&#62;
&#60;p&#62;I have never ever billed Botox in my 14 years of billing and was hoping for feedback?&#60;/p&#62;
